SUBJECT ACCESS REQUEST FORM

If you would like a copy of the personal information we have on you, please fill out the Subject Access Request Form. In accordance with the EU General Data Protection Regulation (GDPR), which takes effect on May 25, 2018, you have the right to obtain this information at this time. Additionally, we will inform you of any processing of your personal data that is occurring, the applicable data retention periods, and any rights to correction, deletion, or limitation of processing that may be applicable.

We will endeavour to respond promptly and in any event within one month of the latest of the following:

• Our receipt of your written request; or
• Our receipt of any further information we may ask you to provide to enable us to comply with your request.

The only uses of the data you provide in this form are to identify the personal data you are requesting and to process your request. You are not required to fill out this form in order to submit a request, but doing so will help us process it more quickly.
